PLEASE NOTE: If your facility already has an account on eRehabData, you will need to contact one of your facility's eRehabData facility administrators to establish a user account for you.

To find out who your eRehabData facility administrators are, go to the eRehabData login screen, type your facility's 6-digit Medicare provider number into the Provider ID box, and click "Contact your eRehabData admin."
